Privacy Policy

Purpose

4Life is committed to the protection of individuals' personal information in accordance with applicable privacy laws. Those privacy laws set out minimum standards for the way that organisations such as 4Life deal with personal information (including health and other sensitive information). Generally, personal information includes information from which an individual's identity is apparent or is reasonably ascertainable.

1. Definitions

1.1. Availability
Data, information and information systems being accessible and useable on a timely basis in the required manner.
1.2. Confidentiality
Data and information being disclosed only to authorised persons, entities and processes at authorised times and in the authorised manner.
1.3. Data
A representation of facts, concepts or instructions in a formalised manner suitable for communication, interpretation or processing by human beings or by automatic means.
1.4. Information privacy
The right of an individual to control the dissemination of personally identifying information about themselves.
1.5. Integrity
The characteristics of data and information being accurate and complete and the preservation of accuracy and completeness.
1.6. Personal information
Any data and information that can be linked to an identifiable person, whether that person is identified directly (eg. by name and address) or indirectly by reference to any combination of attributes which makes possible their identification.
1.7. Staff
A person who is an employee of 4Life Pty Ltd or a 4Life Pty Ltd Franchisee.
1.8. Third party
A person being neither the subject of the data to be disclosed, nor the subject’s health care provider.

2. POLICY

2.1. Collection of Information
4Life only collects personal information which is relevant and necessary in connection with the various aspects of its operations. The purposes for which 4Life collects personal information include:
2.1.1. conducting first aid and personal medical training
2.1.2. issuing financial statements, invoices or receipts in relation to 2.1.1
2.1.3. issuing reminder notices in relation to the expiry of any qualifications or attainments gained in 2.1.12.2. Type of Information Collected
The personal information collected by 4Life will vary according to the purpose for its collection, however, in general this information typically includes:
2.2.1. names, addresses, contact details;
2.2.2. information about staff generated as a result of their employment
2.3. Collection of Information from Third Parties
4Life usually collects the personal information it requires directly from individuals (either by personal contact or by written correspondence). In all cases 4Life collects personal information by lawful and fair means.

Where a participant is receiving training organised by a group or their employer, general contact information may be obtained directly from the organising body.

3. Consent of Disclosure

3.1. Stringent steps are to be applied to ensure the confidentiality, integrity and availability of personal information. The preferred approach is to seek the individual’s consent for all disclosures.

4. Use and Disclosure of Information

SAAS may use and disclose an individual's personal information for the primary purpose for which it was collected, as well as other reasonably expected secondary purposes, where the individual has consented, and otherwise in accordance with applicable privacy laws, including:
4.1. conducting first aid and personal medical training
4.2. issuing financial statements, invoices or receipts in relation to 4.1.1
4.3. issuing reminder notices in relation to the expiry of any qualifications or attainments gained in 4.1.1
4.4. for day-to-day administration and to assist 4Life to manage its services
4.5. to provide information to agents, contractors and service providers engaged to deliver goods and services or otherwise act on behalf of 4Life, or to provide goods and services to 4Life (the identity of which may change from time to time)

5. Transfer of Information Overseas

4Life will only transfer personal information outside of Australia in accordance with applicable privacy laws (including, but not being limited to, with the individual's consent).

6. Data Quality, Storage and Security

6.1. 4Life will undertake all reasonable measures to ensure that personal information it holds is accurate, complete, relevant and up-to-date. All personal information (regardless of its format) is securely stored to safeguard against misuse, loss, unauthorised access, disclosure or modification.
6.2. Personal information is held in electronic form on database systems.
6.3. Access to personal information is restricted in accordance with procedures to personnel whose job functions require access to such information. Certain administrative functions may from time to time be contracted to third parties outside 4Life and in these cases, appropriate security measures are implemented to ensure the security and integrity of all personal information.

7. Government Identifier

4Life does not collect any Commonwealth Government-assigned identifiers (eg; Medicare numbers and pension numbers).

8. Access and Correction

Individuals may request access to personal information held about them by contacting 4Life in writing.
